Risk Survey

If you two or more of the statements below are true, you are at risk for diabetes and should make an appointment to see your primary physician to talk about this survey and your results.

• I am 20% over my ideal body weight.
• I am under 65 years of age and I get little or no exercise during a usual day
• I am over 45 years of age
• I am a woman who has had a baby weighing more than nine pounds at birth or was diagnosed with gestational diabetes
• I have a sister or brother with diabetes
• I have a parent with diabetes
• I am experiencing any of the following symptoms:
     - I have been very thirsty all the time
     - I am urinating more frequently both day and night
     - My appetite has increased although I have been losing weight 
     - I have been very tired lately
     - My eyesight has become increasingly blurry 
     - I have skin infections and yeast vaginitis
     - My feet are painful, numb, and I have a foot sore (ulcer)
