Notice to Physicians

All lab tests ordered for which reimbursement from federally-funded healthcare programs (Medicine and Medi-Cal) is being sought must be medically necessary for the diagnosis or treatment of a patient rather than for screening purposes.

For the laboratory to bill properly and receive payment for tests you have ordered on Medicare beneficiaries the specific ICD-9 or narrative diagnosis(es) must be indicated. It is critical that the diagnosis(es) provided to the laboratory is (are) consistent with those recorded in the patient's medical record on the date of service.
