Options for Getting Started
Option 1
Step 1: Have your primary care physician complete required pre-work which includes:
1) Complete History & Physical
2) Chem 13
3) Lipid Panel
4) Magnesium Level
5) CBC
6) Urinalysis
7) TSH
8) EKG Pre-work must be completed within three months prior to starting the program

There is a consultation fee of $55.00 for our Medical Director to review your forms.
Step 2: Complete your Medical History Questionnaire/Statement of Information/Consent and Commitment Agreement forms; bring to your first appointment.
Step 3: Call 650.482.6065 to set up your first appointment. Please note all pre-work must be delivered prior to your first appointment.
Option 2
Step 1: Call 650.482.6065 to set up your pre-work exams to be performed at the Health & Wellness Center (FEE)
Step2: Complete your Medical History Questionnaire/Statement of Information/Consent and Commitment Agreement forms; bring to your first appointment.
