Pathways Home Health, Hospice & Private Duty has 30 years of experience helping patients and their families decide what type of care is needed for comfort and recuperation. We care for our patients in their home, hospital, nursing home or assisted living facility.
Pathways is a community-based, not-for-profit organization with one mission: to deliver compassionate, high quality health patient and family-centered home health and hospice care that promotes comfort, independence and dignity. Committed to serving the diverse Bay Area community, we have the knowledge and staffing experience to assist your loved one whether they are moving home or to a health care facility.
Last year, Pathways Home Health, Hospice & Private Duty cared for nearly 5,000 home care and hospice patients ranging in age from newborn to 100. Your initial conversation with Pathways will be with an admission nurse who is supported by a team of patient care services. These services include nursing, home health aide, palliative care, hospice medical directors, social work, pharmacy, bereavement, spiritual care, integrative therapies, and an after hours “dream team.”
Medicare, Medi-Cal, HMO’s and Private Insurance often cover our services. When conducting your admission, we can explain what services Medicare and Medi-Cal cover and what services may be covered by private insurance. We are accredited by The Joint Commission and licensed in the State of California.
