A Sequoia Hospital-Dignity Health Affiliate
Dedicated to care with kindness and dignity
Dealing with surgery, a chronic disease, or a life-threatening illness, can be overwhelming. Pathways provides exceptional care at home, giving patients the support they need to achieve their healthcare goals—maybe getting back to bridge at the senior center one day, being pain free to enjoy visitors more, or avoiding the move to a facility.
Our Home Health provides the nursing care and rehabilitation therapy needed to make patients independent in managing their own care. Pathways Home Health prevents hundreds of hospitalizations a year. Check our excellent quality scores on the government website www.homehealthcompare.gov.
We also have a unique Palliative Care program to manage symptoms for people like Bill C. He has terminal cancer and is receiving chemotherapy. Eventually Bill will probably need hospice, but not yet.
Pathways Hospice can’t change the fact that Lena M. is dying—she’s 83 and has end-stage heart disease. But we can do something about pain, about fear, about comfort, about joy, about grief. The specialized and sensitive care of hospice means better quality of life for Lena. That may be the reason hospice patients live longer.
Private Duty aides and Geriatric Care Management mean that George T. can continue to live in his own home. He has Private Duty caregivers three days a week and a geriatric care manager who oversees his wellbeing for his family in Oregon. George feels safe and independent, and his daughter Althea feels peace of mind.
Pathways, a non-profit affiliate of Sequoia Hospital and Dignity Health, has been at the forefront of care at home since 1977. Sequoia Hospital is a co-owner of Pathways Home Health, Hospice & Private Duty. Like Sequoia Hospital, Pathways Home Health & Hospice is accredited by the Joint Commission.
