Advance Health Care Directives
An Advance Health Care Directive allows you to name someone to make health care decisions for you, if you become unable to speak for yourself. In addition, you can give specific instructions about your health care, so that others will know what you want.
You might choose to complete one of our advance directive forms, written in English and Spanish.

Tell Sequoia About Your Health Care Wishes
After you create an advance directive, please tell Sequoia Hospital your wishes.
Bring a copy of your advance directive to the hospital every time you come to the hospital.
