Insurance and Billing
The admitting department is responsible for collecting your insurance/billing information and patient demographics. You may choose to submit this information at the admitting department or by postal mail. If the lead time before admission is short and you are a first time patient at Sequoia, we may call your home to collect medical insurance and personal information pertaining to your admission. It is advised that you bring in your insurance card and/or billing information on the day of admission.
